AzureWave is a limited-panel internal medicine practice. Completing paperwork in advance allows us to
preserve your scheduled appointment time for clinical care.

After scheduling, please download, complete, and bring the forms below. If you do not complete your paperwork in advance, please plan to arrive at least 30 minutes prior to your scheduled appointment time to complete the required forms in the office.
If paperwork is not completed by your scheduled appointment time, your visit may be delayed and may need to be rescheduled.
